Please note: the grant amount does not include GST. If you are successful in gaining a small grant, you will need to provide Hepatitis SA with a tax invoice for the approved amount plus GST, if your organisation is registered for GST.  If your organisation cannot provide a tax invoice, please speak to Cecilia or Jenny about possible alternative arrangements.

Statement of Undertaking

We undertake to provide Hepatitis SA with the following information about the activity supported by the grant, within 30 days of the event:

1. Date of activity
2. Brief description of activity
3. Location of activity
4. Number of participants / brief description of the participants
5. A photograph of the event.
6. A screenshot of your group’s World Hepatitis Day social media promotion  
7. An assessment of whether the event was a success.
8. A simple income and expenditure report, signed and dated by the on my organisation‘s letterhead together with the following statement: "These small grant funds were spent only on activities to raise awareness of viral hepatitis for 2024 World Hepatitis Day."
